NEW SCOUT CHECKLIST

For a Scout to be eligible to attend troop outings,
the following documents must be on record:

  BSA Application
  Medical Form (located on back of BSA application)
  Transfer Application (to be completed by leader of former troop
        if Scout is transferring membership)
  Individual Scout Record (for Boy Scout transfers and Cub Scout
       crossovers)
  Waiver (must be notarized)
  Parent's Insurance Information Form
  Adult Questionnaire
Fees:
New Scout   $55.00
Transferring Scout   $41.00
National Council BSA  $19.00 ($1.00 for transferring Scouts)
Troop Equipment Fee   $25.00
